School Based Services Information

The School Based Youth Services Program is a grant-funded program that offers a broad spectrum of services to students and their families.

The primary goal of the School Based Program is to ensure a successful school experience for students. The program is managed by Hunterdon Medical Center, but is housed at the Hunterdon Central Twilight Program.  Services include supportive counseling, employment and career counseling, assessment, case  management, and referral to appropriate community resources.

The Twilight Progam staff and School Based Counselors provide an opportunity for students to complete their education, obtain skills that lead to employment or additional education, and lead mentally and physically healthy lives.

A School Based consent form, signed by a parent or guardian, is required for School Based Services to be provided. School Based services are confidential. A copy of the Hunterdon Medical Center’s Notice of Privacy Policies (HIPAA) is provided to all students and their families.
