As seniors begin to receive mid-year grade requests from colleges and universities, please use the following procedure:

 

1)  Download and print the 2011-2012 Mid-Year Grade Request Form and fill out the form with the college(s) requesting your grades.

 

2)  Please note that documentation from a college is required.   Attach documentation to the completed form; this can be a printed e-mail, postal letter, printed screenshot showing a request for mid-year grades, or a similar communication from the college.

 

3)  Bring the form, with the attached documentation, to Jean Martinetti or Valerie Setzer in the Counseling Services Department in the 11/12 Building for processing.
