The following forms are required by the Hunterdon Central Health Offices; please download, print, fill out and return these forms, as specified by the Health Offices:

Physical Examination Forms Part A & Part B for 9th Graders, New Students & Athletic Pre-participation Physical Examinations

New Students: Please note that although this form is titled, "New Jersey Department of Education Annual Athletic Pre-Participation Physical Examination Form," it is the correct form to use for 9th grade physicals, new student physicals and Athletic Pre-participation Physicals

Athletes: Please note that per State guidelines, all incomplete  physical evaluation forms (Part B) will be returned and must be completed by the physician (medical home) who performed the physical evaluation. Therefore, prior to submission please review the athletic forms to ensure they are complete.  Any missing information will result in a return visit to the physician and could result in delay of student participation in their chosen sport.
