New Student Physical Forms & Requirements

New Jersey State law requires that each student must be examined at their medical home (family doctor) upon entry into a school district. Therefore, Hunterdon Central requires a physical examination for all Grade 9 students, as well as new students in any grade level. In addition, one physical per year is required for students participating in athletics. Please note that a sports physical will fulfill the requirement for a freshman physical or a new student physical.

The Pre-Participation Physical Evaluation Form must be used for all Grade 9, new student and athletic physicals. Forms can be downloaded and printed from this webpage; please click on the link Health Forms.  Forms can also be picked up in Health Offices, or can be mailed to student homes, upon request.

Grade 9 and new student physicals must be turned into Health Offices by September 30, 2016.  PLEASE NOTE: any students who have not submitted a Grade 9/new student physical to Health Offices by the due date will not be allowed to return to school until the physical has been completed and submitted.

Information on athletic form due dates and procedures can be found on the Athletic Webpage.

If you have any questions about student physicals, please call the 9/10 Health Office:  908-284-7304.
