New Student Physicals

New Jersey State law requires that each student must be examined at their medical home (family doctor) upon entry into a school district. Therefore, Hunterdon Central requires a physical examination for all Grade 9 students, as well as new students in any grade level.

In addition, one physical per year is required for students participating in athletics. Please note that a sports physical will fulfill the requirement for a freshman physical or a new student physical.

The form "Physical Examination Forms Part A & Part B for 9th Graders, New Students & Athletic Pre-participation Physical Examinations" must be used for all Grade 9, new student and athletic physicals. It can be downloaded and printed from the Health Services webpage. Forms can also be picked up in Health Offices, or can be mailed to student homes, upon request.

Grade 9 and new student physicals must be turned into Health Offices by September 30.

Athletic physicals for fall sports must be turned into Health Offices no later than July 16. Forms can also be brought to the Commons on Athletic Submission Nights, on July 18 or July 25.
